We have three separate payment plans, monthly, every 6 months, or annual payment plans. The following two forms are available in PDF format. You will require Adobe Acrobat Reader to open them. You can print out the forms, fill them out, and bring them and a method of payment with you to our office. Both forms are available on location. Some forms may need to be redone onto forms with the correct format, however you can use these tools to make sure you have all the information necessary before visiting our office.

Pricing is done via contract. We have monthly, 6 month, and annual plans. The longer you commit to in your contract, the less your monthly payments will be. However if you sign up for, say, a year, and decide after 6 months you would like to discontinue your training, you are still obligated to make payments up until your year contract has expired. At any time a contract expires, you are eligible to renew your contract or discontinue your participation with CMS.
